Trainer download is going to a 404 page.

Disable/uninstall all AV, firewall and other security software including browser addons and third party apps. Lower browser security. Try different browsers, etc.

methinks the game has been updated, been using the latest trainer for a while now but has recently ceased to work upon the addition of the new 'black death' scenarios.

current trainer now no longer appears to work.
if it was happening before, its still happening now
trainer cannot find game code.

The trainer still works. If you have issues with cannot find game code, verify your steps before pressing the hotkeys. DNA must have been increased and you must click on a country before using the other hotkeys.

Also (re)install .NET framework 4.5

That hasn't been working on my end, and considering I have .NET Framework 4.5 recently installed. All I get are thousands of 'Cannot find Game Code' followed by a 'Trainer Activated' sound every second or so once I press F1

"Thousands of [...]" You should only get ONE message. How often do you press F1 ? And WHEN do you press F1 ?

i think the trainer needs an update, i followed all the proper procedures and the DNA points, Infectivity, Severity and Lethality functions are no longer working, although the increase infected people and dead people functions still work right
