I have 100% for Individuals intel and 93% for organization intel, even though every organization entry is 100%.
I have Alpha Protocol, Al-Samad, Mafia, CIA, G22, Halbech, NSB, Surkov Guard, Triads, U.S. Military, VCI and CSP.
Which one am I missing?

probably a organization that you didn't went across... maybe in next play trough you will find the missing org.

I dont think you will ever do one game and get 100% for everyone and every organisation, some choices have to be made to get more information for each, so executing say Al-Samad may lead you down a different information gathering route than allowing him to live.
And dont trust red heads!
Oh and Don't forget some tuts on ASM and defeating DMA
