ok, ok, this is gonna take some explaining, after all i just got what it is for a few days ago so don't get cunfuzed on me lol
Well all of the Blood stones are in the bonus levels of the Planes after u get they're champion medallion....it can be hard to get them all on adventurous....trust me....VERY HARD....i just got them all TODAY and ive been trying for a WEEK !!!!
lol but its still easier to get them on higher levels like courageous k man? cya i hope i helped!

yes. u must get to division 4. and when u do it'll be on the floor. check out the post on this Plane

Bloodstone Levels: A commonly asked question is where and what are the Bloodstones. There are 11 Bloodstones in the game, when you obtain all 11 Bloodstones, you will be able to enter the second area of the Bloodstone Passage. Inside the Bloodstone passage are a series of 5 consecutive bosses. Defeat these bosses and you will be claimed a true champion which will allow you to pull the sword from the stone in the Plane of Valor.
Here are the level names with the Bloodstone or Item needed:
Warboar Caves - Bloodstone of War The Junkyard Fights - Bloodstone of Innovation Gothic Castle - Bloodstone of Isles 21 Leagues - Bloodstone of Water Hells Cells / Citadel of Pain, Prison - Bloodstone of Torment Crypt of Decay - Bloodstone of Disease Plane of Valor - The Voice of Ralloz Zek sword Isle of Dishonor - Bloodstone of Valor Torment’s Furnace - Bloodstone of Fire Frozen Wind Bunker - Bloodstone of Storms Kelethin - Archeologist Shovel Unexplored Tomb - Bloodstone of Feydark (Archeologist Shovel and Words of Spiritual Harmony required) Plane of Nightmares - Words of Spiritual Harmony Bloodstone Passage - Bloodstone of Nightmares

the location of the bloods stones varies..cuz no matter wat u do when u load a level the locations of the blood stone changes each time... so in order to find the blood stone u have to play the whole level until u find it.. there is a blood stone iv EVERY level... even the one that dont have the side missions..
