Reboot PC to clear excess memory, get a fresh copy of latest trainer and extract it. Run trainer as admin, read the instructions and try again. At times, a new game may be needed to resolve failed options if the data in your save file was altered. Use on a new game and test the option. If it now works, quit to main menu and load your save and try it again.
I tried again, only option 3 is not working for some reason.
I deleted those files indicated above, and tested it in the game's practice mode.

Yes
It's possible they moved some things ONLINE for certain modes of the game, especially modes that allow you to earn XP. No idea.
Options are intended to work with the campaign and in the part of the game you can test stuff (Practice). I've sat an let enemy ships pelt me over and over, and fired missiles endlessly from XWING without running out.
I have no other suggestions.

I'm not using it online, I'm using it in the campaign.
It seems there might also be a difference in the story mode and the training. Option 1 works just fine in training mode, but that same missile will still kill me in the campaign. Maybe my initial finding was correct after all. Certain missiles count as scripted events and will kill you no matter what.
No matter what I do, I can't get the unlimited missiles option to work though, but that's less of an issue...
Unlimited Missiles no longer working for me as well, worked before patch.

I confirm the unlimited missile is not working.
