I did some experimenting with the armor upgrades and the health option. The moment u change the armor from the default the unlimited health stops working, exception seems to be the blood dragon armor. Still have not found the bonus armor to check that one out.
Not sure if it helps, but I can also confirm that option 1 doesn't work during the Collector ship mission (i.e. with the helmet on). Once back aboard the Normandy it works fine. Option 2 works for my squad, but, as with option 1, not myself.
I can't say whether it's the same with just changing the default armour parts, but somehow wearing the helmet definitely buggers things up.
EDIT: Health works using different pieces of armor, but helmet definitely screws health option(s) - the Broken Arrow mission produced the same effects when forced to wear the helmet.

V3 of the trainer has been uploaded which *MAY* fix the health issue in some situations. We cannot test every helmet or armor type that might change the health modifier.

Since the V3 version of the trainer, health DOES NOT work for me anymore :/ Worked flawless before, but I updated since the Trainer Manager recommanded it.
I've reuploaded version 2. We're pretty much done trying to do any additional fixes or make the trainer work for every type of armor/helmet/shoes/underwear or whatever there is in the game as all we seem to do is get more complaints on each attempt.
