does it happen when u start up the game or try and going to the file ??
edit - before i forget sometime your guys will over right the equipment u give them when they ask for a weapon or when u enter town and they have the mats for there old weapon, not happened to me yet but its best to change there equipment bk to there original unless components and money or of no problem(i use trainer so am ok)

yup has happen several times to me. Have to get into the habit of checking everyone's equipment before heading out. Once someone went from a mithril katana to a combat katana n I was wondering why he was not doing too much dmg.
Yes i noticed that happening but i dont think the characters will replace them with noob weaps if you clear your inventory of all weapons as since i did that they dont swap over low end weapons anymore.
There's another thing you can do in that same file, that's allowing so called "mature language" in battles etc. I've tried it out but not played it properly with the change made yet and it works.
There's a line about 20 lines down the page when you open the file in notepad or whatever that reads: bAllowMatureLanguage=FALSE
