i just ran my game from steam this morning with the internet on to make sure everything is current. i ran the trainer and activated from main menu. then i loaded my game and tried all 4 options. they worked immediately again.
not sure what the problem is for you, but this trainer functions for thousands upon thousands who have downloaded it, as well as i just tested it-

He can't make it working on every system till some of your software may block the trainer. There is no software which work 100% on all PC's. GTA IV is best example for it.

GTA 4 runs great on my PC, trainer does it's job as well, I just have to acitivate the unlimited health option twice (on, off and then on again). but besides a few crashes, it works like it should.
But then again, if your software works on 90% of all systems, that's still a great achievement if you ask me. 100% is just impossible. Even Windows doesn't run on 100% of all systems.
