this link gives you all the locations, and remember you have to go all the way through it until it say END FRAGMENT, there is on terminal that doesnt say that but just go through until it completly stops,

no prob, there is one on the level Cortana, but it doesnt matter towards the acheivement, but it does add more to the story, and if you read all the terminals, it gets pretty interesting

ya you have to get the ones on The Ark, and get them in order to understand the story fully

They apparently tell the story of how a Forerunner A.I. betrayed the Forerunners or something along those lines. I just wish that the achievement didn't require you to read all the way through them.
