This isn't an inventory bug (if it were something in your inventory exploding, it would do more than blow your head off, it would blow your arms and legs off too), this is a quest trigger bug.
There is a point later in the game Spoiler:
where you have an explosive slave collar placed on you.
I had the same issue, but after it was removed. Sometimes the game thought it was still there and I would suddenly blow up.
I finished the first part of the brotherhood of steels quest and they removed my exploding collar. haha thats a heck of a bug if it continues to think you have one. hopefully its patched out
