I think that it's Hard mode that it isn't working 100% in.
I have the retail version, no crack being used. I played through Easy and the trainer worked wonderfully. Playing again through Hard mode it's failed me a few times (the other options still work).

Maybe the damage being taken is more than the trainer is ignoring?

same issue here - I have steam version and despite the trainer running the health option does not seem to work. I still take damage from bullets. All the other options work fine.
