{[update]} before you get IFF from reapers do loyalty missions for everyone, than buy all updates for them, all ship upgrades and go than go to claim IFF than all your crew will be collected to collectors base and than don't do any missions go straight to suicide mission and chose the mates for tubes legion or tali, for second team select leader miranda or garrus than for biotic barrier select samara or jack for escort chose mordin or thane and you will get achievement and all crew will be saved!!!

yup, its not just about loyalty - its about skills they have too - not to mention upgrades for the ship

i have loyal legion but he didnt have good enough hacking (i guess) skill so he get a bullet in the head before door were closed

I picked Legion to go into the vents and Garrus to be the team leader and then on my team I picked Tali and Grunt.

Then I picked Samara to be on my team then I picked Miranda.
And For my Two team members I picked Tali and Legion.

I also picked Thane to escort the saved crew members.
All my team members survived.

I have all planets at 100% and have all upgrades.
Illusive Man was ****ed at me cause I destroyed the Collector's Ship.

Ok so here's how I did it, with all loyal squad members and every possible upgrade I could research.
Vents: Tali (also be sure to keep up the pace with her as she moves through the vents)
Second team leader: Garrus (The guy knows his stuff)
Barriers: Samara
Diversion team leader: Garrus
Escort: Mordin

No one died and all the Collectors' base were belong to us, but we set them up the bomb.

Followed your picks for leaders / mission and got a 100% survival rate. Thanks!!

Probably doesn't make a difference, but if I can add another combination which works it might help others in the future. My choices were:

Vents: Legion
2nd team leader: Garrus
Shield: Samara (also used Jack on another playthrough)
Distraction team leader: Miranda
Escort: Grunt

Apart from the times I've used Jack for the shield, I've used that combination in 5 playthroughs and never lost anyone. I've also taken a couple of combinations of people into the final battle with me - Mordin and Legion, Tali and Garrus, Tali and Mordin.

Made sure to have the three main upgrades to the Normandy. All weapon/armour/prototype upgrades I can possibly find (for the most part everything is fully upgraded).

Also had everyone's loyalty and chose "paragon" options in every speech (I say paragon options, but they're not shown as paragon... it just makes more sense to call them so since they're the "hold the line" kind of speech, while the other option is basically a "they ****ed me off so let's go kick some ass" speech).

Not sure if these speech options make a difference to the outcome of who dies or not though - there's quite a lot of discussion as to the possibility, but no one's been able to confirm or deny it yet.

As for timing wise, in order to get everyone's loyalty, do their loyalty missions asap, then go for the reaper IFF. After that mission you'll be able to do Legion's loyalty mission before the Normandy crew is taken. I've also found that you can actually do two missions before the crew is taken - I left Tali's mission so I could take Legion with me to the flotilla and I actually got to do that AND Legion's mission before the collectors came. So maybe Legion's loyalty mission is the trigger... I'm not sure.
