Thats easy. You can tell when hes bout 2 die when he starts walking really slow and he holding his side. And he starts groaning or moaning and like breathing hard. And when hes full of life he walks normal. To get full life again just stand next to a wall and wait for him to lean on it then he will rest which gives him full life again. ( when he stops leaning then he has full life)

I don't think you are actually supposed to open doors at all in the game
