The locations of the seven Orichalcum+ses are:

Space Paranoids - Central Computer Mesa (chest)
Twilight Town - Sunset Terrace (as Sora) (chest)
The World That Never Was - The Brink of Despair (chest)***
100 Acre Wood - Finish Starry Hill
Atlantica - Finish "A New Day is Dawning"
Olympus Coliseum - Beat Goddess of Fate Cup
Any Moogle Shop - Get at least one of EVERY other material

That wasn't so hard, was it?

***I should state that the Orichalcum found here is hugging the wall of a building on a narrow ledge next to the pit below the Org's castle. So you might not notice it the first time you go through. It is NOT the large treasure chest.

Note: THERE IS NO ULTIMA WEAPON+.

UPDATE: Well, this seems to happen a lot, though I don't know why people don't realize it at the time, realize it after going back, or why it isn't given directly to the moogle in the first place...

When you get the Orichalcum+ prize for obtaining one of every material, YOU MUST LEAVE THE MOOGLE SHOP AND RETURN TO SUBMIT IT TO THE MOOGLE. If you don't do so, even if you have all the other necessary materials, the Ultima Weapon cannot be made. OK? Good.

I would have sent you a PM but there wasn't enough room. There are not 13 Orichalcum+ in the game. You have to level up your moogle to reduce the amount of Orichalcum+ needed and substitute the O+ with some other Synthesis items.
