The line which seems to be incorrect is "This form is not to be used to request support for additional distributions of the game, please use the official trainer thread to make those requests."
When I made such a request on the official trainer thread for the game, a site support staff member indicated that such a request required going through the "additional options" request form.
Since the additional options form doesn't indicate that either, perhaps both should have their text updated to indicate that as the proper place to make those requests and that it's limited to Lifetime Plus members only.
Just a suggestion, so as to limit others from having similar confusions about additional platform support requests.
