1) Run the game first
2) Run the trainer as admin
3) DO NOT ACTIVATE WITH F1
4) Disable the internet driver or disconnect - it means UNPLUGGED.
5) Load the game - they will be in offline mode
6) Activate with F1 then use any options
7) You need to remind OFFLINE until you quit the game to desktop
That's not hard to do it.
The physical disk is obsolete and Uplay on PC can run without need any physical disk, so it don't going to make any difference, but console like PS4 and Xbox One need physical disk if you don't own digitally, so for PC, it is 100% digitally.

The game can be put in offline mode, can't disable EAC though without crashing said game for whatever reason (Even in offline mode which is weird.) so what that software does in the background could indeed a bit of a risk though seeing how there's bots and what not for For Honor it doesn't seem to be all that perfect even if it's very strict with restricting access to the software.
But yeah the recent problem with game bans whether it's Call of Duty or the Forza or a game requiring always online like For Honor well it's no wonder you're being cautious and taking measures to prevent this even if it requires fully disconnecting from the net to ensure that there won't be any traffic from the game or EAC.
It's not that much work though I do hope the game developers update the game so EAC can be disabled and having it work similarly to say Watch_Dogs 2, offline without having to put the entire UPlay client into offline mode and maybe firewall the exe just to be sure.
So. I have test the trainer (on windows 7). No problem. The game work fine, the trainer work fine too.

P.S : I have a question : As i have cheat in solo and offline mode only to unlock all skills, can i play in online mode without trainer and not to be banned please ?

The only way to find out is to try it for yourself.
Does EAC still log what you do or upload anything to their servers even though you have your internet "unplugged"?
Since the service is still running even when you're offline, I'm curious if they still log any information you do offline, and then send the info online once you restart the game in online mode to get into the main lobby so you then unplug your internet to use the trainer again.
I tested it, and everything I did offline affected my MP profile as soon as I started the game up (main lobby) and didn't join any sessions or went "in-game".
I checked my Ghost Recon Network profile and believe it or not, as soon as I restarted the game in order to use the trainer offline. My profile was uploaded to the website.
Also to my knowledge they will do a mass ban wave this friday as they have done with both Watch Dogs 2 and For Honor.
I think Ubi Soft can answer your question better as of what is logged and what is passed on to their servers.
