I have scoured far and wide to make sure of this.
Stupidly I had built a nuke in the game for the FOB.
What I didn't know was that doing that gives you an astonishing amount of evil level points...50,000. It's the biggest, most evil thing you can do in the game far and away. This is one of numerous aspects of the game that were poorly documented. You can't even track your evil level in the game.
The reset evil function on the trainer seemingly cannot overcome this even though my nuke has long since been dismantled.
If I use it I show up at Mother Base all cleaned up...I do everything properly and everything looks good... I take a shower for grins...and as soon as I'm back on the ACC it's like it all never happened.
I have used this reset evil function numerous times in the past with no issues so there is something about building the nuke that puts something over the edge. Just something to be aware of.
I also note that the trainer no longer affects heroism levels on or offline, at least on my end.

I've been wondering how the game is treating the Evil business.
I'm playing offline so can't make Nukes. But I just went on the White Mamba mission. I didn't even take a lethal weapon and I killed nobody. Yet back on MBase here comes the blood and horn. I made a stealthy run except for wrestling with the commander kid.
It still showered off but I dunno where it came from. That was almost a no contact mission. I was making a speed run for my 'S' rating and didn't even take the time to Tranq the other kids.

You can see far and away making a nuke is the most evil thing you can do. +50,000 right there which makes you Satan incarnate instantly.
You have to eliminate ten nukes just to get back 30,000 and still be 20,000 in the hole.

If you're not far into the game, prepare yourself for a mission titled 'Code Talker' ... as a Quiet fan I think you will enjoy that.
