I've played through twice with same character and have only found 1 orange item. I'm just starting over on a third playthough and just wondering, where's all these items?

Bosses, Slot Machines, Skag;Bullymong;Stalker piles, Civilian Lockers, Hyperion Lockers, Rare red loot chests, Regular Enemies, Quest Rewards.

The drop chance for them is pretty low outside of some special cases like the class mod Terramorphous drops but as above you can acquire them from many sources but again the chance to find a unique item is often very low even from bosses that do have a special item tied to them (Like the grenade mod from Boom & Bewn) so outside of using slot machines repeatedly or finding a way to hack the item discovery chance or what that vault relic increased you might not find that many unique items.
(The more players there is also increases enemy amount and variety such as more higher level creatures which in turn also increases the chance for better loot.)

If I remember correctly, the BradyGames guide states that the chance of finding an orange item dropping from a Badass+ is about 1 in 1,000. From any other source the chance is about 1 in 10,000+. That's a LOT of playing.

Correction: "You're much more likely to get legendary items off of certain foes who have a much-increased chance--1-in-20 instead of 1-in-thousands--to drop a particular legendary item". Sorry for the lapse in memory.

One thing to keep in mind is that Loot on BL2 is randomly generated.

The way items are generated is nothing compared to how BL1 was.
Randy and the other devs did this on purpose to make things more "rare".

It took till my 3rd character for me to get an orange or 2 by normal means.

1 was from knuckle dragger at the start, the other from Terra with 4 ppl.

Having all 4 people with the VH Relic should help with things until someone can modify the loot drop % table.
