Complete the Phil Bell mission Truck Hustle.

Get into the car with Jimmy and his men. Get up on the roof to watch over the meeting with a sniper rifle. When the deal goes bad, take out as many as you can with the sniper rifle, then head down to protect Jimmy. Go after the car that tries to get away with Jimmy's money. Chase them and kill them then pick up the money. Now drive Jimmy home to complete the mission.
Requirements to unlock:
Complete the Jimmy Pegorino mission Pegorino's Pride.
Reward:

Drive to the marker on your map. Once there, Pavano's men will spot you and try to get away. Follow them to the rest of Pavano's men. Once there, kill all of them. Drop your wanted level to complete the mission.

Complete the Jimmy Pegorino mission Payback.

After the cut-scene with Jimmy, get a car and drive to the hospital. Walk inside without showing any weapon and head to the back of the ER. Two cops will guard the end of the hall. Kill them and run into the last
room and kill the cop there as well. Kill the cops that shows up outside the rooml, then kill Anthony. Fight your way out of the hospital and get away any way you can. Drop the wanted level to complete the mission.
Requirements to unlock:
Complete the Jimmy Pegorino mission Flatline.
Complete the Phil Bell mission To Live and Die in Aldernay.

Drive to Jimmy's house and attend the meeting with Ray and Phil. After the meeting, leave the house. After a while, Jimmy will call you telling you to kill Ray. Drive to the marker on your map. Ray gets into a car with his bodyguards. Follow the car. You have to take out the car and kill Ray and his bodyguards before he reaches his destination.
