I start the game on normal, and after completion if there is anything worth getting like trophies I re-play on the hardest difficulty.

I will play easy first to learn the controls and know about the story. Then I will go play the harder difficulty.

It depends really, if it's on, say a Play Station or an XBOX I would go for normal, or even hard, because I'm more confident with the controls. But if it's on a PC, I usually start on easy, because I'm not as used to using the keys for controls, although I usually get the hang of it in a while.
