I've managed to get Q4 installed on my Windows 7 64-bit system by actually finagling the DVD's autorun program to run in WinXP compatibility, and I've installed the v1.4.2 point release and can get the game running, but I'm fearful of random crashes that may or may not happen. Are there any custom (i.e. unofficial, fanmade) patches out there that bolster Q4's compatibility with Win7?

For anyone else interested Quake IV runs fine on Windows 7 X64. Just install it in WinXP compatibility mode as predcon did. Download the latest patch, 1.4.2 which adds multi-core support.

It's a little more than just "install in WinXP compatibility mode". When the disc attempts to execute Autoplay, you'll get an error message about it. Open your Task Manager, go to Processes and right click on the Autorun (or it might have been Autoplay, or even just Installshield, basically, if it looks like it has to do with installing or setup, it's a safe bet that's the one you want) and click Properties, then set it to WinXP compatibility mode Service pack 2. Eject your disc and put it back in, and the splash menu should load up just fine. The same process works for the Bonus Disc if you have the Special Edition DVD set.

Mine installed a little differently. When I got the error message I got another windows message telling me there was an error or didn't install, something like that and then asked me if I wanted for windows to check for solutions. I clicked yes and it said to install in windows XP compatibility mode and I said yes and it installed no problems. I didn't have to remove the disk and insert it again.

Either way I'm glad it works with no problems or crashes.
