In first play through I lost jack at the battle with the human reaper.
In second play through I lost Zaeed at the battle on the collector ship.
How can I save everyone?

im also interested i havent finished it yet but i dont want to lost anyone for ME3

You have to purchase all the upgrades to your ship and have full loyalty for everyone. You may also have to buy all "upgrades" for your team members (example: Legion's sniper or grunt's shotgun) however I am not sure of that.

Make sure you have all Upgrades on your ship before you go to the Collector's base, and your teammates loyalty.

Then When your in there, who you choose to lead team, shield also very very important. Here is my suggestion:

For Vents : Tali or Legion

Team 1: Miranda or Garrus

Team 2 Miranda or Garrus

Escort: Jacob

Barrier: Samara or Morinth

I strongly suggest for Team 1 and Team 2 stick with the same leader.

Many people have had issue with Mordin dying alot, so i suggest taking him with you.

In my final battle I used:

Vents: Garrus
Team 1: Jacob
Team 2: Miranda
Escort: None :P
Barrier: Jack

And I didn't have a single death. (Other than escort failure)

So I think it does depend on who you take, however I think the most important is upgrades/loyalty.

all upgrades and all loyalties.

Vents: Tali
Team leader: Garrus both times
Escort: Mordin
Barrier: Jack

No one died for me.
