If someone can provide a savegame with exact recreate instructions where both options 1 and 2 do not work, we'll be happy to look at it.
I have a save from the Steam version where pressing '2' gives 100000 health to only one of my squad mates, and pressing '1' gives 100000 health to the other. Shepherd stays at his original health. (I'm guessing this is an "off by 1" bug?)
This has happened to me on both V1 and V2 trainers.
I think I may know why. If you have any DLC like armor and equip it, it may cause the Unl health to not work. The Recon Helmet is one example. I was using the N7 armor by default and the Unl health worked fine, but the moment i equipped the Recon Helmet and went into a mission, the Unl Health stopped working.
Only promo item I've used is the collector's rifle. Haven't had any issues until I reached the collector's ship mission. Now neither the unl. health or squad health seems to be working.
The unlimted health option for Shepard stopped working for me on the collector ship mission. I did change the 'upper torso' part of the armour..may have to change it back and see if that is what caused it.
We have yet to receive a working, non-DLC savegame with instructions on replicating the issue. No matter how many times or people say that it doesn't work, without a savegame, we will never be able to look further into it.

It worked, you guys are onto something at least with regards to changing armour. I changed the armour back to the default armour and the unlimited health is working again..so far anyway.
