I was always under the impression that if you are running the trainer in Campaign, and only join local servers, it's fine if you're with friends. At least that's what I did in the first l4d and now the second, and it's fine.

Of course it doesn't work online. CHU says, in multiple places around the site, they do not condone online cheating. And neither should anyone else. It ruins the fun for anyone else playing.

I have been using the trainer since the game came out, not all the time but alot. Now as for getting baned the way to avoid that in multi-player mode is by using local area server they alow cheats, just as in l4d 1. I have never been kicked out of a game using cheats or the trainer in either game or banned from either. As for telling someone you are using cheats or a trainer thats just letting them know what type of game they are playing. As you are the hosting person on the server. In other words your the boss and if they do not like it they can goto another server or get kicked by you and your friends who like the way the game is setup if they act up. I have done this several times with people who did not like the way the game was setup and most of the time they are trying to kill you and your friends and cussing you and your friends out. So all this talk about getting banned I have never worried about it, also getting banned while trainer is activeacted and joining a game has never got me kicked or banned. Remember you have to hoist the game in order for the trainer to work! If you are not hoisting it you fall under the guidelines of the deadedcated server and your trainer is null and void. This is from my experience playing L4d 1 and L4d 2. I have over 680 games in L4D and over 100 in L4d 2.
