To make GTAIV remember your controlls, locate "LaunchGTAIV.exe" and run it as an administrator, set the controls the way you liker, then exit. then start the game normally by the social club launcher and you will have your controls ^^

Why would the game forget your controls?
I've not had that issue but will keep this tip in mind
Only using CH trainers to add fun to games already completed or for testing mods, etc.
