Do achievements still unlock when using the trainer?

yep they do
if you mean new cars, clothes, designs and so on.....

you can also get the stars to unlock new scenarios .

Yes they will, provided you are properly logged into an 'online' live account (which you will be most likely if you provided a serial at some point) Some might think this is 'naughty' to use a trainer for profile achievements, but yes, it won't effect them
