The capability of your graphics card (core, shader processors, ROPs, etc.) isn't very good, as your card is very low end.
GTA IV is also extremely CPU strenuous, considering how much AI there is to control. You'll need a good quad core to pump up the AI vehicle settings.

His gfx card isn't the worst around. I have a 8600GT with 256 GDDR3 and I run my game on medium. The 3450 series are from last year series. It's only under the 48xx series.
Tried GTA IV on my HTCP (AMD Athlon 5000+, 7950GT, 2GB RAM)
Also it was running ok when you turn all background programms off.
And that trick helped me to play 1920x1080 without any problems with that trick in my other post.
When i tried it without that trick i was not able to play 1024x768.
So it should run on your PC too without any problems.

The X1650 is old. The HD3450 is about a year old and is still pretty good.
It should be enough for GTA 4 but not for other games.
3450 is more for multimedia & office but not for games.
Even 3450 is pretty old you can get 4450 for same price.
Better you should get 4670 it is bit slower than 48xx but still good and not that expensive.
Or get GeForce 9600GT/GTS.
But best for you is get more RAM and upgrade your GPU after it,because RAM costs lesser and give much performance not just for games.
