It's a program that organizes all your trainers into a nice and neat list for you to use for your games. It's the difference from a nice list to doubleclick on your trainer vs navigating all the folders to your trainer in Windows Explorer.

You start it and see if there is newer version of trainer.
You can download trainer with it but you have to unpack and paste the trainer into the folder of trainer manager to get it work. You also can change the keys with the trainer manager.
Little Suggestion: Would be nice if it would be possible to download trainers with trainer manager and to get it directly into the folder of the manager xD
