at a guess by first launcher screen i would think you mean the pop up launcher that says multi/single player or exit, thats just the launcher, choose single player, go through the , i think its 59 million intel/nvidia adverts get to main menu in the full screen game which has like singleplayer, settings, credits and exit as options and press f1 there, then hit single player and make a character.
if you didn't mean that, then maybe you are using the new trainer with the unpatched game and its not recognising it or patched game old trainer.
For anyone having any issues, please tell us about your install. Retail, Normal, Collector's Editon, Digital Download, DX9, DX10, 32-bit, 64-bit, etc. As you can see there are many different variables here and of course it's not going to work for every variation across the board. Perhaps with the feedback from you guys we can at least determine things in common on those having problems.
I know Cal tested the new skill points option with every char type and it was working fine in our normal retail 32-bit dx9 version.

bah, one issue seems to be that our trick of simply copying over the x86 files into the folder from the disc, which worked pre patch, seems to be loading me a game without a main menu now
guessing the patcher only patched the 64 bit folders, which is not hugely useful to, lol.
guess i'm going to uninstall, reinstall the x86 version if possible and then try it both pre/post patch.
before the patch using vista 64, with default 64bit game install and 32bit folders off the disc the game ran fine in 32bit mode. it seemed the trainer worked completely fine with first character, first level up a bunch of skill and attribute points were added completely fine. used them up within a few more level ups(from actually playing) and then i could add attribute points fine as long as i had 1 when i added them. but skill points i could not get any more of , probably ran out level 4 or 5 and from there couldn't get any more than standard 1 per level up.
2nd character i tried i couldn't even seem to get the skill points to work with the first level, only pressing the button after i had 1 skill point in there so not a clue, while for that character every other option worked as it should.
got it working finally , the only thing i hadn't tried was loading my game then running the trainer, works for me but its a bit hit and miss weather it saves the info when i log out
