as far as i know its random but if your aiming for a certain sex you can always save right before she/he gives birth, you know when they are waving their aims around and taking deep breaths, when you unpause the game and its not what you want, dont save, go back to your neighborhood, re-enter the lot and see what you get this time, this way can be kinda annoying but its works!
