I played a couple hours and the graphics are off the chart. Even a blinding sun when walking into it, effects are fluid and battle is fairly straight forward. Wish I had a trainer for some of the harder ones, but the storyline is excellent.

Must be playing a different game to me lol. Running everything on Ultra, and while i wouldn't say it's an ugly game, it's far from 'off the charts'

I confirm that the optimization and graphism are very good. I have a 780 Ti and i play to this game in extreme details. No lag, no crash, no specific problem except a little clipping sometime.

Sadly, besides 60 FPS, I got a very strange micro-lag impression that also bugged me in the Witcher 2. Got and R9 280x (not crossfire). Somehow the movement appears choppy.

I get that when I have vsync off on the witcher games with a monitor on 120hz+

The game looks okay. GTA V, Dragon Age Inquisition (Although this might be a preferance, but I thought the environments were far less "clustered and messy," it was really easy to tell where you were and the colors were brighter?) look way better. The E3 2013 Demo was where my socks were blown off, but, just like Watch_Dogs, what was promised and what was delivered were two separate things.
I don't mind. Every game sorta looks like it. Shadow of Mordor kinda comes close. I'm playing Ultra everything on 1080p and 4k as well.

4k is not really playable for me (780 SLI). Have to turn down settings to medium to have a lag free game. This is the first game where I had to turn down a lot of settings.

I've had to turn a lot of settings down a bit, far more than I thought I would, honestly. Still looks good. Damn good. Peoples faces especially are some of the best I've seen. In terms of the immersion this allows, for me to sink myself into the world presented to me, it's more than sufficient, even on my aging rig. I don't know if it's me having lower standards than some folk, or of it's some folk making a mountain out of a molehill (Though I assume it's a mixture of the two).
If you can't dazzle them with brilliance, baffle them with bull****
