I had a similar experience with fall, my game crashed when I fell from a ledge. I guess if you take one hit damage that exceeds your health limit the game crashes if you have unlimited health enabled.

Edit: OK regarding that crash, I played till Market station without using the trainer at all with a new game. I can buy the extended shotgun and mod it without the game crashing. I guess the trainer corrupts the save somehow as the game still crashes if you continue the game without the trainer. A fresh new game is the only way to go.
