Has anyone else had an issue where enemies spawn with about 40% hp? doesn't happen with all enemies just a select few, also this started happening when i got into mid/lategame of New Game+ makes some missions impossible, say if i have to escort a vehicle it spawns with 20% hp and dies in 2 shots...

Turn off unlimited health in the trainer and try again, seems it lowers health of other stuff in the process. (At least that's my experience with it, I'm fairly certain it was the health option.)
