I've played through twice with same character and have only found 1 orange item. I'm just starting over on a third playthough and just wondering, where's all these items?

The drop chance for them is pretty low outside of some special cases like the class mod Terramorphous drops but as above you can acquire them from many sources but again the chance to find a unique item is often very low even from bosses that do have a special item tied to them (Like the grenade mod from Boom & Bewn) so outside of using slot machines repeatedly or finding a way to hack the item discovery chance or what that vault relic increased you might not find that many unique items. (The more players there is also increases enemy amount and variety such as more higher level creatures which in turn also increases the chance for better loot.)
If I remember correctly, the BradyGames guide states that the chance of finding an orange item dropping from a Badass+ is about 1 in 1,000. From any other source the chance is about 1 in 10,000+. That's a LOT of playing.

Correction: "You're much more likely to get legendary items off of certain foes who have a much-increased chance--1-in-20 instead of 1-in-thousands--to drop a particular legendary item". Sorry for the lapse in memory.
