This is the part right after Indecent Purposal Quest, where you talk to the leader of Foatsom.
You follow Tris to the Prisoner's Barge and she asks you to help her with your Sign to heal the prisoner.
I can't figure out what to do. When I select the Sign and use "Q" key I draw the weapon and the guards attacks me.
How do you use a sign without entering combat?

Approach the guy and you'll see a prompt at the bottom to use the left mouse button to cast the sign.
