at right the start of the game, i can use the abilty trainer cheats to get all abilities, but how do i get and choose specialization at the very start of the game....?

You have to wait until you are settled in at home in kirkwall before you can pick a specialization.

another question is, with the trainer, can i choose all 3 specializations ? or just 2

you can get all 3 with the trainer. Toggle #8 I believe will give you 10 Specialization Points. You only need 3 but having the extra 7 won't matter. You use Ability Points to fill in the specialization abilities once you've opened them up.
